Sunscreen permission form

Attached is the permission form for students to have sunscreen while at school. Students are only allowed to bring sunscreen for personal use, and cannot share with other students. Please instruct your student on how to apply the sunscreen before bringing it to school, and no spray sunscreen is allowed.
